Case Management Support at Rise N Thrive provides structured coordination for families navigating OhioRISE services and related systems. Offered through collaborative communication and administrative guidance, this service helps ensure care activities remain aligned with authorized plans, documented needs, and care team direction. Our role is to support clarity, organization, and continuity so families can confidently engage in their child’s services.
Youth with complex behavioral health needs often interact with multiple providers, systems, and requirements that can be difficult to manage alone. Case Management Support focuses on coordination, information flow, and accountability to help services function smoothly within approved parameters. All coordination is conducted in accordance with program standards, documentation requirements, and established care team protocols to maintain consistency and compliance.

Families meet with one of our licensed therapists for an initial evaluation of behavioral health concerns. Assessments typically last 60 to 90 minutes.
